Before you can start your clinical placement in week 5, you must have completed all the required documentation.  If any is outstanding, you may fail the course as attendance at clinical placement is compulsory.  Instructions for completing each item.

10.  Compulsory Documentation

10.1  Queensland Health Documentation

Queensland Health requires that you carefully read some important information and complete relevant requirements prior to clinical practice.

This documentation is to be completed by all Students regardless of whether clinical placement is undertaken at a private or public clinical school site.

There are THREE forms to be returned to the School of Nursing, Midwifery and Social Work:

At the completion of the OHS Module request a certificate of completion be sent to your email address, print one copy for your records (the clinical school or Queensland Health may ask you to provide this at any time) and one copy for the School of Nursing and Midwifery.
This is a mandatory requirement and it will require a substantial amount of time to read all the information

Please note that by completing the 10.4: Immunisation Guidelines, 10.5: Blue card application and 10.6 National Police Certificate you will also be meeting the Queensland Health requirements – you are still required to read the information in these sections on the Queensland Health website

10.2  Fitness to Undertake Clinical Practice Questionnaire

The Fitness to Undertake Clinical Practice Questionnaire should be returned in an envelope addressed to the Undergraduate Program Director and marked confidential either in person or at the School office:

As per the documentation, if during the course of your degree there is a change in your health that detrimentally affects or is likely to detrimentally affect your capacity to complete the clinical components of your degree you MUST notify the Program Director immediately.

10.3  Apply First Aid Certificate

A current Australian Apply First Aid Certificate is compulsory for all students. This certificate must remain valid for the entire duration of your program. Apply First Aid certificates are valid for three years from the date of initial training.

Please note Students are not reimbursed for the costs of acquiring the Apply First Aid Certificate.

10.4  Immunisation

The relevant immunisations must be completed and the signed form returned prior to commencement of clinical practice. Students who receive credit for clinical courses must also return this paperwork by 16 February 2015. 
 
Midwifery and Dual Degree Students – PLEASE NOTE – Students will not be able to attend clinical practice in exposure prone areas (for example, birthing suites) until Hepatitis B serology is completed and returned.

10.5  Working with Children Check – Blue Card Application

Please complete the application form and RETURN to the School of Nursing, Midwifery and Social Work with appropriate forms of identification (see page three).

10.6  National Police Certificate

You will need to obtain a National Police Certificate as this is required by some providers where you undertake clinical practice. This certificate costs $52.75 and is an Australia wide check of criminal history records.
 
If you live in Queensland this certificate can be obtained by completing the application form and presenting it at your nearest Police Station with appropriate forms of identification – please see top of form.

Please include certified copies of identification and a money order or cheque for $52.75 payable to the Commissioner of Police, ensuring your signature is witnessed at the bottom of the form. No scanned, faxed or emailed copies of the application will be accepted – please do not forward the application form to the School of Nursing, Midwifery and Social Work. The turnaround time for Police Certificates is approximately 3-5 weeks.
 
Please retain your certificate when you receive it, we are not permitted to hold the original or a copy on file. When required we will ask that you present the certificate to the clinical practice provider. If the certificate cannot be presented at the appropriate time it will mean you will be unable to undertake clinical practice, and may fail to meet the requirements of the program.

10.7  Uniform Orders and Delivery

It is recommended that you purchase at least two UQ School of Nursing, Midwifery and Social Work polo shirts to wear while you are on clinical practice (orders can only be placed at the beginning of the year). Please see the order form for details. Orders must be placed and paid in full no later than 20 February 2015. The sizing kits for uniforms will not be available at the St Lucia Campus until Monday 19 January.
 
Please note you will not be able to commence your clinical placement unless you have the required uniform shirt.
 
Orders will be delivered to and collected by Students from the School of Nursing, Midwifery and Social Work. Any orders not collected within three months of the delivery date will be returned to stock and all payments will be forfeited. Orders cannot be changed or cancelled. Please note: all prices are correct at the time of publication, however, may be subject to change.
 
When you are on clinical practice, you must wear:
 A freshly laundered UQ School of Nursing, Midwifery and Social Work shirt
Your program/year level badge
Smart black trousers, shorts or skirt with the uniform shirt (denim jeans are not acceptable)
Black fully enclosed shoes (in order to comply with occupational health policies)

Both forms need to be returned to the school office.  Bring two forms of identification.
